For a clinic that takes 300 calls a month, an AI phone answering service costs about USD 129 to 750 a month on the plans three US vendors publish. A fully human answering plan for the same 300 calls costs USD 2,100. The price is the easy part. The real work is deciding which calls the AI may take, signing the privacy paperwork and measuring the results before the front desk relies on it.

What does an AI answering service cost a clinic per month?

We read these prices on the vendors' own pages on 29 October 2026. The example clinic sends 300 calls a month to the service, after hours and when the front desk is busy. Each call lasts about 3 minutes, so 900 minutes in total.

Rosie charges USD 49 for 250 minutes, USD 149 for 1,000 minutes and USD 299 for 2,000 minutes. Booking straight into the calendar and live transfers start on the USD 149 plan. Goodcall charges by unique caller: USD 79 for 100, USD 129 for 500 and USD 299 for 2,000. It compares itself with traditional answering services, which it puts at USD 1.50 to 1.90 a minute. Smith.ai starts its AI calls at USD 1.60, hybrid calls at USD 2.50 and human calls at USD 7. A call handed to a person costs USD 3 extra. Its human plans run from USD 300 for 30 calls (USD 11.50 for each extra call) to USD 2,100 for 300 calls.

How do you set up an AI answering service for a clinic, step by step?

  1. Count the calls. Take two weeks of the phone system log and sort each call by reason: booking, rescheduling or cancelling, hours and directions, prescription refills, test results, billing, and symptoms.
  2. Give the AI the simple calls only. Booking, rescheduling and opening-hours questions follow a script. Refills, results and anything about symptoms go to a person or become a message for the clinical team. A caller who describes an emergency is told at once to hang up and call the emergency number.
  3. Start with after hours and overflow. Leave the main daytime line alone until the numbers are in.
  4. Choose the price model that matches your calls. Minute plans suit short calls. Per-caller plans suit patients who call more than once. Per-call pricing suits low volume.
  5. Run two weeks in shadow mode. Staff place test calls and a staff member listens to every recording before real patients reach the AI.
  6. Measure for 60 days. Track the share of calls finished without a person, wrong bookings, complaints and transfers. Make a go or no-go decision on your own numbers, not the vendor's.

Does an AI answering service need a HIPAA business associate agreement?

In the US, it does if the service handles patient information. The American Academy of Pediatrics lists answering services among typical business associates: outside firms that use or disclose protected health information for a practice. None of the three pricing pages above mentions HIPAA. Get a signed agreement before the first real call, and ask where recordings and transcripts are stored and for how long. In the EU, Article 50 of the AI Act has required since 2 August 2026 that people are told when they are talking to an AI, unless that is obvious. Put the disclosure in the first sentence of the greeting in every market.

How much work stays with the front desk?

Smith.ai's own page says most firms start with its hybrid option, where people stay on the line as backup. Every handoff to a person costs extra: if 60 of the 300 calls go to a person at USD 3 each, add USD 180 a month to the AI plan. None of the three vendors publishes how many calls end without a person, so the share you measure in your own trial is the only number you can count on.
